Funding to support the development of an interdisciplinary blueprint for care coordination with emerging health care delivery systems. A one-day conference on care coordination will examine two emerging and interlocking areas to be addressed as new models of care within health care reform. The papers presented at this meeting and the issues that emerge from the discussion will be compiled, along with highlights of a literature review on care coordination including the latest thinking from each participating discipline, into a conference pack to inform an invitation only roundtable with key stakeholders and experts from government. The roundtable is designed to produce recommendations for interdisciplinary care coordination policy and the implications for health professional training.
